Following a monthslong investigation by Minneapolis, Minn., Tegna station KARE11, the state has moved to shut down a housing program marred by allegations of fraud and abuse. “For months, KARE 11’s Recovery Inc. series has revealed evidence of forged signatures, phantom services, and millions in suspicious Medicaid billing by multiple HSS providers,” the station said. “Some clients said they never even met anyone from the companies billing in their names.” The state is working on redesigning and relaunching the program with stronger fraud prevention and accountability.
